Graduation Achievement Charter High School (GACHS) ceased operations on September 30, 2018.

If you are a former GACHS student or family member and need to request items from your student record, please fill out the electronic form below. If you are unable to complete this form online, please call 404-656-2837 to request a copy of the student records request form to be mailed or emailed to you.

Requests will be fulfilled within ten (10) business days of receipt.

Please note: the SCSC is unable to change any items in a student’s record. This includes but is not limited to amending a student’s transcript, attendance record, or disciplinary record. The SCSC is only able to produce copies of GACHS student records.

If you select 'Parent or Guardian,' you represent that you are authorized to receive these records because you are a parent or legal guardian of the student you are requesting records on behalf of, and he/she is dependent according to Section 152 of the Internal Revenue Code if the student is over eighteen years of age. 'School Administration' and 'Military' should attach a student consent form in the Identification Verification box or other form of Verification.
The SCSC must verify your identity before completing your records request. You may either 1) upload a copy of the student's or legal guardian's state issued identification OR 2) upload a copy of this (https://scsc.georgia.gov/sites/scsc.georgia.gov/files/related_files/document/Student%20Records%20Request_Affidavit.pdf) completed, notarized affidavit attesting that you are entitled to receive copies of the student’s records. \n

SCSC will absorb the cost of mailing transcripts, but SCSC requests that all full student records be received via email.
